Below are answers to some of the questions you may have.
A. You can request your medical records using the form below and the following guidelines, requirements:
Please allow for all online requests to be processed within 24 hours. If you need your records sooner than 24 hours, please call our medical records team directly at 443-569-8760.
Be sure to include the location, date and time you would like to pick up the records.
All pick up requests require a photo ID to be shown.
If you would like someone else to pick up your records on your behalf, this person will need to be listed on your HIPAA form (below).
If you are anything other than the patient please upload a medical release (if this is an option).
A. You can access your information such as medical records and reports through our Patient Portal. Please visit:
CMI Radiology Portal To receive copies of your images onto a disc from our imaging sites, simply visit any location with your photo ID. Upon arrival, the images will be printed for you. Please allocate 15-20 minutes for the processing time. To avoid a wait please call 855-455-8900 option 3 to submit a request through our imaging records department. Alternatively, if you prefer to have your images mailed to you, we can accommodate that request as well. Please note that the mailing process takes 24 hours for processing, and standard delivery will be utilized.
A. Due to HIPPA regulations, we are unable to provide your results over the phone.
A: The HIPAA Privacy Rule establishes national standards to protect individuals' medical records and other individually identifiable health information (collectively defined as “protected health information”).
A: Effective immediately, Chesapeake Medical Imaging is using the Clearpath platform to fulfill and process all legal/insurance requests (subpoenas and authorizations).
